Rewording is narrower: changing individual words and clauses while keeping the same structure and meaning. In practice people use the terms interchangeably, but rewording tends to stay closer to the original sentence.
Not by itself. It becomes a problem when it's used to disguise someone else's work as your own, or to dodge a citation. If the ideas aren't yours, rewording them doesn't change that. You still need to credit the source.
