Can therapists write without cloud processing?

Typeahead processes nearby text with a local model on the Mac, so clinician-authored writing does not travel to an AI vendor for completion. In a supported writing field, Typeahead can suggest the next few words at the cursor, and you stay the author by accepting a suggestion with Tab.

That changes a specific part of the data flow. A draft can receive useful completion without being sent to an AI cloud for inference. It does not decide whether a destination app such as Microsoft Word, the Mac, a backup service or an account is appropriate for patient information.

Those remain separate decisions, governed by the practice’s actual legal, ethical and security obligations. Processing location is therefore a concrete property to inspect before adopting the tool.

Use local autocomplete where you already write

In a supported field, the writing stays in place. Begin the sentence, pause at the cursor and review an inline suggestion if one appears. Press Tab if it matches what you meant. Keep typing if it does not.

After using Typeahead daily, we notice the useful moment is often after beginning a sentence about the plan for a later session, when the thought is settled but the familiar wording has not reached the field.

Chat is a heavy interface for the next few words. It asks you to leave the sentence, describe a task and bring an answer back. A chat tool is the better choice when you want a generated outline or a back-and-forth response. Typeahead completes sentences you started, in your voice, at the point where you are already writing.

Local processing changes one data relationship

HIPAA coverage does not attach to a therapist or practice solely because it delivers health care. It covers health plans, clearinghouses and health care providers that conduct specified financial or administrative transactions electronically. HHS describes which organizations must comply, while more protective state provisions may also apply. A practice may therefore need to assess federal rules, state law and professional duties together.

HIPAA does not prohibit cloud processing. A cloud provider that creates, receives, maintains or transmits electronic protected health information for a covered entity is generally a business associate and requires an appropriate business associate agreement. That status can apply even when the information is encrypted and the provider does not hold the key. HHS explains the cloud relationship and BAA requirement.

The label on a clinical document also matters. Under HIPAA, psychotherapy notes are separately maintained notes that document or analyze counseling conversations. The definition excludes diagnoses, treatment plans, symptoms, prognosis and progress summaries. HHS explains the category and its additional authorization protections.

Applying the psychotherapy-note label too broadly can obscure which rule applies to which information.

Keeping AI completion on the Mac removes the AI vendor’s cloud-processing path for that writing. It narrows one privacy risk. It does not establish compliance for the practice or determine how another app handles the completed text. That narrower claim is still consequential for clinicians who cannot add an AI cloud disclosure path to sensitive drafts, a concern we examine further in keeping client-related drafts off cloud AI.

Test the path, then review the workflow

You can verify local completion after license activation and the one-time model download. Use a blank test sentence rather than patient information for the check.

1. Turn off the Mac’s network connection.
2. Open a supported app and the writing field you expect to use.
3. Begin a sentence and pause at the cursor to see whether a suggestion appears.
4. If one appears, press Tab and confirm that the suggestion enters the field.

A successful test shows that completion can run without a live connection. It does not test the destination app’s storage, synchronization or access controls. It also does not test device security, backups, account permissions or the practice’s process for retaining and disposing of records.

For regulated entities, the HIPAA Security Rule requires risk assessment and administrative, physical and technical safeguards, including protections for workstations. Professional ethics reinforce that review independently. APA Standards 4.01 and 6.02 require reasonable precautions with confidential information and protection across the creation, storage, access, transfer and disposal of records. ACA Sections H.2 and H.5 address confidentiality limits, electronic transmissions and electronic-record security.

The adoption review should name the information being written, the law and professional code that apply, the destination app, who can access the Mac, where copies are stored and how records leave service. Then make the bounded decision: use local processing to remove the AI cloud path, and assess the destination app, Mac, backups, accounts and record-handling workflow under the privacy, security and professional rules that apply to the practice.
